Imagine two people, X and Y, in a relationship. Scenario 1. X and Y are shopping at the supermarket and at one point X chooses a personal use object (epilator, razor, toothbrush, etc.). Y offers to pay for the object saying that it's X's birthday in a week and that is the present. Scenario 2. Y plans a surprise for X's birthday. When the day comes Y gives X the birthday present.

When my partner plans a surprise birthday present for me, it shows they care.
I prefer getting my birthday present like in Scenario 1.
There is no difference between the two scenarios, a present is a present.
